Understanding Your Vascular Health

The Whole Circulatory System, Not Just the Veins

Why This Evaluation Matters

The same disease process that damages vein valves can also affect the arteries, and venous care is rarely the whole picture.

Atherosclerosis is the gradual buildup of plaque inside arterial walls, and it is a systemic process, meaning it tends to affect more than one part of the body at once. A patient with narrowing in the leg arteries is statistically more likely to also have narrowing in the arteries supplying the heart or the brain. These conditions can develop silently for years, often without symptoms, until they do not. Recognizing that possibility is part of what a vascular surgeon’s training adds to a venous evaluation.

A broader vascular look is often worthwhile when one or more of the following apply:

  • A history of smoking, current or past
  • High blood pressure or high cholesterol
  • Diabetes
  • A leg wound that is slow to heal
  • Leg cramping, aching, or fatigue that comes on with walking and eases with rest
  • A family history of heart attack or stroke

Three Territories, One Underlying Process

What Dr. Schmetterer Evaluates

Because the circulatory system is connected, a thorough evaluation sometimes looks beyond the veins themselves, especially when a patient's history points that way. One thing Dr. Schmetterer may check for is atherosclerosis, a gradual narrowing of the arteries that tends to show up in the legs, the heart, and the neck.

In the legs, this narrowing can cause cramping or tiredness when walking that eases with rest, sometimes called intermittent claudication, a pattern Dr. Schmetterer can recognize and screen for with a simple, painless test called an ankle-brachial index. Because the same process tends to affect more than one area of the body at once, patients with this kind of leg artery narrowing are sometimes at higher risk for related issues in the arteries feeding the heart or the brain. When that’s the case, he can identify the risk early and coordinate with the patient’s cardiologist or primary care provider as needed. Looking at the legs, heart, and neck together, rather than any one on its own, gives a fuller picture of a patient’s vascular health.

The good news is that medication often plays a meaningful role, sometimes enough on its own. For leg artery narrowing, certain medications can ease cramping and help patients walk further and more comfortably. For venous ulcers, other medications can reduce pain and speed healing when combined with treatment of the underlying vein issue. If a finding does call for more than medication, such as a minimally invasive procedure or surgery, Dr. Schmetterer talks through the options in plain language and coordinates with the right specialist.

A Lifetime Spent on the Whole Vascular System

Dr. Schmetterer's Philosophy of Care

This kind of evaluation reflects the full arc of Dr. Schmetterer's career, not just its most recent chapter.

Thirty-four years spent operating on hearts, lungs, and arteries leaves a particular habit of mind: never looking at one vessel without considering what it might say about the rest of the system. That habit shapes every venous consultation. Many patients come in focused on aching legs or visible veins and leave with a clearer understanding of their overall vascular health than they have had in years, sometimes with a referral that turns out to matter more than the original visit. None of this changes the focus of the practice, which remains venous care; it simply means that focus comes with a wider field of view.

The goal, as with everything here, is preventing the complications that come from things going unnoticed: a wound that will not heal, a heart attack, a stroke. Catching them early is often the difference between a conversation and an emergency.

FAQ

Frequently Asked Questions

Why would a vein doctor evaluate my arteries?

Venous and arterial disease often share the same underlying cause, atherosclerosis, and frequently occur in the same patient. Dr. Schmetterer’s training spans both, so a venous evaluation can also identify arterial issues, such as peripheral arterial disease in a non-healing wound, that would otherwise go unnoticed. Catching these early can be the difference between a wound that heals and one that does not, or a stroke that is prevented and one that is not.

Atherosclerosis is the gradual buildup of plaque, made up of cholesterol, calcium, and inflammatory cells, inside the walls of arteries. It is the underlying cause of most heart attacks, strokes, and peripheral artery disease, and it is a systemic process that often affects several arteries at once. Risk factors include smoking, high blood pressure, high cholesterol, diabetes, and a family history of heart attack or stroke.

Yes, leg cramping, aching, or fatigue that comes on with walking and improves with rest is a classic sign of intermittent claudication, caused by reduced blood flow to the leg muscles during activity. It is diagnosable with a simple, painless test called the ankle-brachial index, often performed at the time of consultation. Many patients respond well to medication and lifestyle changes, and a vascular evaluation can clarify the best path forward.

Dr. Schmetterer is professional and kind. Before the procedures I had an appointment with Dr. Schmetterer to explain the course of treatment . When the treatments were completed he met with me to evaluate the procedures and to discuss directions for aftercare. It was very easy to make appointments. I went to another doctor in the area and I won’t return . I didn’t have an appointment with him before or after the procedures ; I met with his P.A. . When he came into the room he didn’t introduce himself or even say Hi . He never asked me how I was feeling and left the room without saying Goodbye.

I can’t say enough good things about Dr. Shmetterer and the entire team. I had 6 varicose veins treated, and going into it I’ll be honest I’m not great with anything involving veins or blood. I was pretty anxious at first. But from start to finish, they made the entire process smooth, comfortable, and way less intimidating than I expected. Dr. Shmetterer was professional, confident, and took the time to explain everything in a way that actually put me at ease. The treatments themselves were handled with precision and care, and the staff was incredibly supportive the whole time. What really stood out was how they made someone like me who normally struggles with this kind of stuff feel completely under control and taken care of. That says a lot. If you’re dealing with varicose veins and even a little hesitant like I was, this is 100% the place to go. Couldn’t recommend them more.
