Venous Disease

Chronic Venous Insufficiency

When Blood Flow Loses Its Way

Understanding What Is Happening Beneath the Skin

Chronic venous insufficiency develops when the veins of the legs are no longer able to return blood efficiently to the heart.

Healthy veins contain tiny one-way valves that keep blood moving upward against gravity. When those valves are damaged, whether by age, genetics, pregnancy, years of standing, or a prior blood clot, blood begins to flow backward and pool in the lower legs. The pressure that builds is what patients eventually feel, and see, at the surface.

Symptoms typically worsen as the day goes on and improve with rest or elevation. The most common include:

  • Leg heaviness, aching, or fatigue, especially after long periods of standing or sitting
  • Swelling around the ankles that improves overnight
  • Visible varicose veins or clusters of surface veins
  • Throbbing, burning, or restless discomfort in the legs
  • Skin changes near the ankle, such as darkening, dryness, thickening, or itching
  • Wounds near the ankle that are slow to heal

Many patients have spent years assuming these symptoms are simply part of getting older. They are not. Chronic venous insufficiency is a diagnosable, treatable vascular condition, and identifying it early typically makes treatment simpler and more effective.

Finding the Source, Restoring the Flow

Diagnosing and Treating Chronic Venous Insufficiency

Effective treatment begins with finding exactly where, and why, the venous system is failing.

The cornerstone of that evaluation is a duplex ultrasound, a painless imaging study that maps the veins of the leg and shows which valves are leaking and how far the condition has progressed. At the Youngstown office, these studies are performed by two dedicated vascular ultrasound technicians with years of focused experience. The ultrasound is scheduled as a follow-up appointment after the initial consultation, and Dr. Schmetterer reviews the findings with each patient at that visit. In most cases, patients leave the consultation with a clear picture of next steps and a treatment plan established after the ultrasound results are reviewed.

Treatment is then matched to the anatomy and severity of each patient’s condition. Options include radiofrequency ablation, which closes the diseased vein with controlled heat; VenaSeal, which seals it with a medical adhesive and requires no heat at all; Varithena foam sclerotherapy for larger or more complex varicose veins; sclerotherapy for surface veins; and compression therapy, either as primary support or after a procedure. Every procedure is performed in the office, without a hospital stay or general anesthesia, and most patients return to normal activity within a day or two. Medicare, Medicaid, and most insurance plans cover treatment when medical necessity is documented.

Care Shaped by a Lifetime in Vascular Medicine

Dr. Schmetterer's Philosophy of Care

Treating venous insufficiency well requires understanding the entire circulatory system, not just the veins visible at the surface.

Dr. Schmetterer brings more than 34 years of cardiothoracic and vascular surgical experience to every evaluation. That background shapes how he approaches this condition: mapping the full venous system before recommending anything, sequencing treatment so each step builds on the last, and exercising the judgment to know when a vein needs a procedure and when it simply needs monitoring. The goal is never to treat what is visible. It is to correct what is causing it.

For his patients, the measure of success is not the ultrasound report. It is being able to get through a long day on your feet, take the walk through Mill Creek Park, and enjoy the everyday life of the Valley without heavy, aching legs. Follow-up visits and imaging confirm that results hold over time, because protecting long-term vascular health is part of the treatment, not an afterthought.

FAQ

Frequently Asked Questions

What causes varicose veins?

Varicose veins are caused by weakened or damaged valves in the veins that allow blood to pool instead of flowing efficiently back to the heart.

Most modern varicose vein treatments involve minimal discomfort, often using local anesthesia to keep patients comfortable throughout the procedure.

Many patients can return to normal daily activities within a day or two, depending on the specific treatment and individual recovery.

Dr. Schmetterer is professional and kind. Before the procedures I had an appointment with Dr. Schmetterer to explain the course of treatment . When the treatments were completed he met with me to evaluate the procedures and to discuss directions for aftercare. It was very easy to make appointments. I went to another doctor in the area and I won’t return . I didn’t have an appointment with him before or after the procedures ; I met with his P.A. . When he came into the room he didn’t introduce himself or even say Hi . He never asked me how I was feeling and left the room without saying Goodbye.

I can’t say enough good things about Dr. Shmetterer and the entire team. I had 6 varicose veins treated, and going into it I’ll be honest I’m not great with anything involving veins or blood. I was pretty anxious at first. But from start to finish, they made the entire process smooth, comfortable, and way less intimidating than I expected. Dr. Shmetterer was professional, confident, and took the time to explain everything in a way that actually put me at ease. The treatments themselves were handled with precision and care, and the staff was incredibly supportive the whole time. What really stood out was how they made someone like me who normally struggles with this kind of stuff feel completely under control and taken care of. That says a lot. If you’re dealing with varicose veins and even a little hesitant like I was, this is 100% the place to go. Couldn’t recommend them more.
