Lymphedema and Phlebolymphedema Management

When Swelling Doesn't Go Away

Understanding Lymphedema and Phlebolymphedema

Lymphedema is persistent swelling, most often in the legs, caused by damage or dysfunction in the lymphatic system.

Unlike the temporary swelling that follows a long day on your feet, true lymphedema does not resolve overnight or with elevation. It is a chronic condition, and without structured management it tends to progress, affecting the skin, mobility, and daily comfort. In a venous practice, the form seen most often is phlebolymphedema: years of untreated venous insufficiency gradually overload the lymphatic system until it can no longer keep up. The two systems are anatomically intertwined, and when one fails, the other carries the burden. Certain signs suggest swelling may be lymphedema rather than ordinary fluid retention:
  • Swelling that persists overnight and does not improve with elevation
  • A feeling of firmness, tightness, or heaviness in the affected limb
  • Skin that has thickened or changed in texture over time
  • One leg noticeably more swollen than the other
  • Repeated skin infections in the swollen area
  • A long history of varicose veins or untreated venous disease

Distinguishing between the two matters, because each calls for a different plan. A vascular evaluation can tell them apart and identify whether a treatable venous problem is driving the swelling.

A Program, Not a Procedure

How Lymphedema Is Managed

Lymphedema is managed rather than fixed in a single visit, and being clear about that from the start is part of good care.

The gold standard is Complete Decongestive Therapy, a structured program delivered by physical therapists who specialize in lymphatic disease. It combines manual lymphatic drainage, a specialized massage technique that redirects fluid; multilayer compression bandaging between sessions; therapeutic exercise that activates the muscle pump; and meticulous skin care to prevent infection. The practice connects patients with local therapy teams experienced in this work, and helps each patient understand that the program typically moves through an intensive phase followed by long-term maintenance.

Where Dr. Schmetterer’s role becomes decisive is the venous side. In phlebolymphedema, venous insufficiency is the engine behind the lymphatic overload, and treating it, typically with endovenous ablation or sclerotherapy guided by a duplex ultrasound, reduces the pressure the lymphatic system has been fighting against. It does not replace therapy, but it can meaningfully improve what therapy achieves and lighten the maintenance that follows. For patients with residual swelling between visits, a home pneumatic compression device such as the Nimbl, delivered and set up at home through Tactile Medical, can become a comfortable part of the long-term routine.

Care That Supports the Whole Picture

Dr. Schmetterer's Philosophy of Care

Honest care for lymphedema starts with saying clearly what helps, what doesn't, and who does what.

Dr. Schmetterer’s part is the vascular evaluation and the treatment of the venous disease so often hiding beneath the swelling, a connection his decades across the entire circulatory system prepared him to recognize. The specialized therapy belongs to the therapists, and he coordinates with them rather than duplicating them, so each patient gets a plan where every piece reinforces the others. Patients are never handed a diagnosis and left to navigate it; they leave understanding their condition and exactly what comes next.

The goal, as always, is measured in daily life: shoes that fit again, legs that feel lighter through a full day, the confidence to stay active and independent. Lymphedema asks for consistency, but with the right plan and the right team, most patients find the swelling, and their routines, far more manageable than they feared.

FAQ

Frequently Asked Questions

How is lymphedema different from ordinary leg swelling?

Ordinary swelling from venous congestion or long periods of standing typically improves overnight or with elevation, while lymphedema persists and tends to worsen over time without active management. The difference comes down to cause: lymphedema involves a structural dysfunction of the lymphatic system rather than a temporary fluid shift. A vascular evaluation can distinguish between the two and guide the right treatment plan.

Phlebolymphedema is swelling caused by both systems failing together: long-standing venous insufficiency progressively overloads and damages the lymphatic system. It is one of the most common forms of lymphedema seen in a vascular practice. Treating the underlying venous disease is a critical part of management, working alongside the structured therapy that addresses the lymphatic side.

Lymphedema is typically a chronic condition, but its symptoms can be managed effectively and many patients achieve significant, lasting reduction in swelling. The combination of complete decongestive therapy, treatment of any underlying venous disease, and a consistent home maintenance routine is what protects those results. With the right plan, most patients regain comfort, mobility, and confidence in their daily activities.

Dr. Schmetterer is professional and kind. Before the procedures I had an appointment with Dr. Schmetterer to explain the course of treatment . When the treatments were completed he met with me to evaluate the procedures and to discuss directions for aftercare. It was very easy to make appointments. I went to another doctor in the area and I won’t return . I didn’t have an appointment with him before or after the procedures ; I met with his P.A. . When he came into the room he didn’t introduce himself or even say Hi . He never asked me how I was feeling and left the room without saying Goodbye.

I can’t say enough good things about Dr. Shmetterer and the entire team. I had 6 varicose veins treated, and going into it I’ll be honest I’m not great with anything involving veins or blood. I was pretty anxious at first. But from start to finish, they made the entire process smooth, comfortable, and way less intimidating than I expected. Dr. Shmetterer was professional, confident, and took the time to explain everything in a way that actually put me at ease. The treatments themselves were handled with precision and care, and the staff was incredibly supportive the whole time. What really stood out was how they made someone like me who normally struggles with this kind of stuff feel completely under control and taken care of. That says a lot. If you’re dealing with varicose veins and even a little hesitant like I was, this is 100% the place to go. Couldn’t recommend them more.
