Radiofrequency Ablation (RFA)

Modern Vein Relief, Lasting Comfort

What Is Radiofrequency Ablation?

Radiofrequency ablation is a minimally invasive, in-office procedure that permanently closes diseased leg veins using controlled thermal energy.

It is the modern standard of care for venous insufficiency affecting the great and small saphenous veins, the main venous highways of the leg, and it replaced vein stripping as the preferred treatment for most patients. Candidates are typically patients whose duplex ultrasound shows reflux in these larger veins and whose symptoms, heaviness, aching, swelling, or skin changes, have persisted despite conservative measures such as compression. A thin catheter delivers heat to the vein wall from the inside, causing it to collapse and seal permanently. Once closed, blood reroutes naturally through healthy neighboring veins, relieving the pressure behind the symptoms.

A few practical facts make RFA easy to plan around:

  • Procedure time: under one hour, in the Youngstown office
  • Anesthesia: tumescent local anesthesia; the office prescribes EMLA numbing cream to apply to the skin before arriving; patients who are very anxious may also receive a mild sedative before the procedure
  • Driving: most patients drive themselves home; those who have been prescribed a mild sedative before the procedure should arrange a ride in advance
  • Return to activity: the same day, including work and low-impact exercise
  • Follow-up: an ultrasound within several days confirms the vein is sealed

Medicare, Medicaid, and most private plans cover RFA when venous insufficiency is documented by ultrasound and conservative measures have been tried, with coverage verified before scheduling. For many patients, RFA addresses the largest source of reflux and sets the stage for any further treatment that follows.

Precision, Guided in Real Time

How RFA Works and What Recovery Looks Like

The entire procedure is guided from start to finish by real-time ultrasound, performed by the practice's two dedicated vascular ultrasound technicians.

A thin catheter is positioned inside the diseased vein under that guidance, and tumescent local anesthesia is placed around the vein to keep the patient comfortable and protect the surrounding tissue. The office prescribes EMLA cream to apply to the skin before the appointment, and for patients who are particularly anxious, a mild sedative may be prescribed as well; those patients should arrange a ride rather than driving themselves. The catheter then delivers radiofrequency energy that heats the vein wall in a controlled manner, causing it to collapse and seal. Most patients describe the sensation as pressure rather than pain, and the procedure itself is completed well within the hour. A compression bandage is applied before the patient leaves the office.

Compression plays an important role afterward: a dressing, wrap, and bolster stay in place for the first 48 hours, stockings are worn day and night for the next two weeks, and daytime compression continues for about six months for the best long-term result. Short, frequent walks are encouraged from day one, since calf movement supports circulation as the vein closes. For four weeks, heat and direct sun on the treated area are avoided, and anti-inflammatory medications give way to acetaminophen, since the body’s own controlled inflammation is part of how the vein is fully reabsorbed. The complete day-by-day protocol is outlined on the Compression & Post-Procedure Care page.

A Technology Chosen with Judgment

Dr. Schmetterer's Philosophy of Care

Every technology in this practice is there because Dr. Schmetterer chose it deliberately, and RFA is a clear example.

Among heat-based vein treatments, he uses radiofrequency rather than laser, because the controlled, lower-temperature energy of RFA typically means less post-procedure discomfort, less injury to surrounding tissue, and a smoother recovery. It is the kind of decision that comes from decades of treating the circulatory system: the question is never which device is newest, but which approach serves this patient’s anatomy, comfort, and long-term result best. That same judgment determines when RFA is the right tool at all, and when VenaSeal, Varithena, or simple monitoring serves the patient better. Each recommendation comes with the reasoning behind it, explained plainly, so patients understand not just what is being done but why.

The aim of all of it is unglamorous and important: legs that carry you through a full workday, an evening walk, and a wedding reception in the Valley without the ache that used to come with them. That is the comfort RFA is meant to restore.

FAQ

Frequently Asked Questions

Is radiofrequency ablation painful?

RFA is performed under tumescent local anesthesia and is very well tolerated by most patients. The office prescribes EMLA numbing cream to apply before arriving, which helps minimize discomfort at the start of the procedure. Some pressure may be felt during catheter placement, and mild bruising or soreness along the treated vein is common for one to two weeks afterward. Acetaminophen is typically all that is needed during recovery, since anti-inflammatory medications can interfere with the healing process that follows treatment.

The treated vein is permanently closed and does not reopen. New venous reflux, the backward flow of blood caused by valve failure, can develop in other vessels over time, particularly in patients with a genetic predisposition. A follow-up ultrasound confirms each result, and periodic monitoring is what protects long-term comfort and mobility.

Vein stripping is an open surgical operation requiring general anesthesia, a hospital setting, and weeks of recovery. RFA achieves the same goal, closing the diseased vein, through a small catheter during an office visit of under an hour, with patients driving themselves home and returning to normal activity the same day. Long-term outcomes are equivalent or better, which is why RFA replaced stripping as the standard of care.

Dr. Schmetterer is professional and kind. Before the procedures I had an appointment with Dr. Schmetterer to explain the course of treatment . When the treatments were completed he met with me to evaluate the procedures and to discuss directions for aftercare. It was very easy to make appointments. I went to another doctor in the area and I won’t return . I didn’t have an appointment with him before or after the procedures ; I met with his P.A. . When he came into the room he didn’t introduce himself or even say Hi . He never asked me how I was feeling and left the room without saying Goodbye.

I can’t say enough good things about Dr. Shmetterer and the entire team. I had 6 varicose veins treated, and going into it I’ll be honest I’m not great with anything involving veins or blood. I was pretty anxious at first. But from start to finish, they made the entire process smooth, comfortable, and way less intimidating than I expected. Dr. Shmetterer was professional, confident, and took the time to explain everything in a way that actually put me at ease. The treatments themselves were handled with precision and care, and the staff was incredibly supportive the whole time. What really stood out was how they made someone like me who normally struggles with this kind of stuff feel completely under control and taken care of. That says a lot. If you’re dealing with varicose veins and even a little hesitant like I was, this is 100% the place to go. Couldn’t recommend them more.
